3. Code of Conduct - please read carefully - this is referred to in the registration form and all members and their families must behave within these rules

4. Registration Form - to be completed for all members regardless of whether one was completed in previous years.

5. Parental consent letter covering Colts playing adult cricket.
   • Children under 10 will not be eligible to play adult matches and children in the U14 age group or older do not need written parental consent.
   • If your child is between 10 and 13 and you wish your child to be considered for playing in any or our adult teams you must therefore return a signed copy of this letter.
   • Receipt of this letter does not in any way guarantee that your child will be asked to play, but no letter will rule him/her out of selection.
   • In line with ECB policy, decisions as to who should play within these age groups will be taken in conjunction with our Head Coach, Pete Williams.

6. Parental Consent form covering photography and changing rooms.
   • If you gave your written consent in both of these areas last year and are happy for the permission to stand then you do not need to complete this form again.

7. 50/50 Club form - the newsletter provides details about the 50/50 club - please support the club in this simple way - every little really does help. Please note that for security purposes the form on the web site does not show the account details. Please obtain the relevant details from a Colts organiser, senior playing member or committee member.

8. Whitsun Coaching Clinic registration form - please return this promptly if you wish to book a place on this successful course.
